This program is a contemporary, physician-conducted and monitored approach to weight loss, based on current research and the clinical experience of Dr. Diaz. The program focuses on lifestyle changes, which eventually will help patients keep off unwanted or extra pounds.

The goal of the HD Weight Loss Program is twofold: To help patients break poor eating habits and gain new healthy eating habits. Most people who begin a weight loss program regain their pre-diet weight or may even put on more pounds. For this reason, this program is conceived as a plan intended to break eating habits that produce these repeated unwanted results and to achieve ideal body mass index (BMI).

The program is intended to reach out to those who are medically considered to be either overweight or obese and who are medically stable to enroll in this weight loss program. Conditions such as diabetes, hypertension and heart disease should be under control before starting the program.

The program may also be used by certain patients to better control medical conditions like hypertension, diabetes, glucose intolerance, metabolic syndrome and high cholesterol. This program is not intended to be a treatment or cure of any of these medical conditions. If patients suffer from any of these medical conditions or any other health problems, Dr. Diaz will consult with your primary care physician to see if it is medically acceptable for you to enroll in this weight loss program.

Patients with uncontrolled diabetes, unsuitable hypertension, heart disease, renal failure, neuropathy, rheumatologic diseases and immune-compromised patients are generally not good candidates for this program.

Part 1: Medical Evaluation

The first part of this program is a thorough medical evaluation which includes a physical exam and medical history with a comprehensive metabolic and nutritional evaluation. Components of the evaluation include a screening test for thyroid disease, diabetes or glucose intolerance, hypercholesterolemia, renal function test, liver function test, total proteins, albumin level, electrocardiogram, heart rate monitoring, blood pressure monitoring, oxygen saturation, body mass index (BMI) and percentage of body fat.

Based on the results of these medical evaluations, you and Dr. Diaz will discuss and set realistic and achievable loss weight goals.

The program is designed to be conducted under close medical supervision with Dr. Diaz for an initial 3 month period. It takes about 40 days to break a bad habit and providing further time for adjustment and reinforcement these new habits will allow you to continue to live with these new and healthy lifestyle changes. Close follow-up with Dr. Diaz during the initial 3 month period will also allow you to personalize the program, to adjust it to your daily routine and make it a new and permanent habit. Follow-up appointments with Dr. Diaz will be scheduled after the initial consultation at Week number 2, 5, 9 and 12, for a total of five office visits.  

The use of appetite suppressant medications will also be discussed during the medical evaluation. Medications may be prescribed if needed after you understand the possible side effects and contraindications, based on the literature and research available. Appetite suppressant medications are effective only for short periods of time to help patients to engage in healthy eating habits.

Part 2: Food and Nutrition Knowledge

The second part of this program is designed to increase your knowledge about nutrition and food. When you know better you generally do better. You are also required to spend time reading and mentally absorbing the material provided and to understand fundamental principles of healthy eating habits.

This section of the program will guide you though the understanding of basic topics such as the body mass index, food labels, correct portion sizes, the balanced 40/30/30 diet concept, caloric counting, and high and low glycemic index foods. In addition, guidance on how to buy groceries with a list of healthier products, including the seasonings and spices to add flavor to your food, as well as tips for the healthy eating will be discussed.

Program concepts that are not well understood should be discussed for clarification with Dr. Diaz in the first follow-up appointment. This will help ensure the adherence to program goals and achievement of objectives.

Part 3: Menu Design

The third part of this program includes menu design for breakfast, lunch and dinner. Menus will be designed to be prepared in an easy way and with minimal time required. Snack menus, alternative menus, advise when eating out, tips on how to entertain at home and lunch alternatives for the busy people who can not bring their lunch to work will be provided.

Part 4: Exercise Programs

The final section of the program is dedicated to the exercise tips. General education about exercise as well as type and amount of exercise suitable for your weight loss condition and lifestyle to will discussed.

Each person may need a personalized exercise regimen based on individual needs, interest, physical limitations, lifestyle and limiting medical conditions. Your exercise plan will be prescribed by Dr. Diaz after the complete medical and nutritional evaluation. 

It is the goal of this program that you will be able to continue with these new lifestyle changes on your own and without physician assistance. For some individuals, optional periodic medical supervision may be advised after finishing the program to ensure that the weight loss remains permanent.
